For Staff, Faculty and Administration
- All persons who will not be on contract during the following college year must obtain form #2736-A NPC Check-out Form (see Forms section) from the Human Resources Office.
- It is the responsibility of the individual who is leaving to obtain authorized signatures on form #2736-A from each of the following listed offices:
- Campus or Center Office (keys, credit cards, furniture)
- Division director or immediate supervisor (curriculum materials, instructional materials, program software)
- Library (desk copies, AV equip. and accessories, library materials, inter-library materials)
- District library
- Campus or center library
- Business Office (new address, outstanding travel advances, invoices or requisitions outstanding, equipment, and credit/telephone cards)
- Payroll Office (current and/or future address and telephone numbers, state retirement funds, COBRA insurance option)
- Bookstore (book order for next year)
- Appropriate vice president (special assignment materials, performance objectives, program coordination duties)
- Registrar's Office (final grades/incompletes)
- Vice President for Learning and Student Services (faculty clearance)
- The Human Resources Office (certification status, current and future address and telephone number, letter of resignation)
- Information Services
- The completed form #2736-A must be turned into the Human Resources Office before the last paycheck will be released.
